John was admitted to Life Care Center of Stoneham following an acute stroke that caused expressive aphasia and muscle weakness on his right side.
Upon admission, John required total assistance with bathing, dressing, and toileting tasks. He needed help with bed transfers and bed mobility. He could walk only short distances, with a brace on his right lower leg and the assistance of two caregivers.
John underwent physical, occupational, and speech therapy five times a week to address his impairments in motor control, strength, balance, coordination, short-term memory, attention, executive functioning skills, and expressive language.
After just two months, John was independent with bed mobility, transfers, and ambulation, no longer needing a brace. For bed mobility and transfers, he needed only some setup assistance.
He transitioned to assisted living for ongoing support. His ultimate goal is to return home fully independent, and we are confident that he will!

Ms. Heider was admitted to Life Care Center of Morgan County for short-term therapy and to receive IV antibiotics for treatment of cellulitis.
At the time, Ms. Heider was at a supervision level because of her poor activity tolerance and her low endurance. She received physical and occupational therapy to build her endurance and enable her to complete activities of daily living and walk greater distances.
During therapy, she learned to use a rollator to help her walk farther. She also learned various breathing techniques to elevate her endurance. After working hard every day, she was able to return home to her daughter.
Ms. Heider says that her stay at the facility was good, and she appreciates the care and assistance she received to help her reach her goal of returning home independent.

After suffering from a severe stroke, Susan was admitted to an acute care rehab facility for treatment. But after her therapy there reached a plateau, she was transferred to Hallmark Nursing Center for long-term care and further rehabilitation.
When she arrived at Hallmark Nursing, Susan was confused and easily frustrated by her poor communication ability. She disliked all types of activity and preferred to stay in bed. She required complete assistance with all areas of functionality, including bed transfers, dressing, feeding, and wheelchair propulsion.
During her stay, Susan received treatment for severe communication problems and severe swallowing difficulty. Physical and occupational therapy focused on improving her ability to preform activities of daily living. Nurses worked on healing her wounds with frequent dressing changes and close monitoring of her skin.
Susan has made great progress when it comes to her physical ability and overall mood. She now trusts and recognizes her caregivers with a smile on her face. She now asks to go to the gym and work with the restorative nursing aids. To stay motivated during her exercises, she listens to 80s rock music. She has made new friends and enjoys having meals with them in the dining room.
Susan still receives occasional therapy but is now able to eat independently and walk over 100 feet with assistance. Her wounds have fully healed.
