L..T has been a resident at Life Care Center of Grandview since 2021.
Before her short-term therapy program at Grandview, she required a wheelchair and moderate assistance for all mobility and basic tasks, including bed mobility and transfers. She also needed total assistance with dressing due to limitations from pain in her knees and her shoulders.
L.T. received physical and occupational therapies three times a week, addressing the pain limiting her mobility. Both therapy disciplines focused on improving her strength and range of motion to restore her functional mobility and ability to complete activities of daily living independently.
After meeting her milestones, L.T discharged with the ability to complete all functional mobility tasks with modified independence and to walk with a four-wheeled walker for short distances. She is currently participating in a walk-to-dine program to extend her walking distances and using a wheelchair for longer stretches.

After his hospitalization for congestive heart failure and hypoventilation syndrome, Mike went to Life Care Center of Longmont for short-term rehabilitation.
When Mike arrived at the facility, he was only able to walk less than 200 feet with contact guard assistance and struggled with balance, strength, and self-care. Additionally, Mike showed signs of mild cognitive deficits.
Mike received weekly sessions of physical, occupational, and speech therapies. During this time, he focused on improving his strength and mobility and increasing his ability to walk longer distances without assistance.
After just 30 days, Mike discharged from therapy with the ability to perform daily activities and walk over 500 feet on his own, with the use of a cane and 4-wheeled walker.
Once at home, he will receive in-home therapy services to augment his recovery and help him stay on the path of independence.

Connie came to Life Care Center of Elkhorn after she received hospital treatment for deep-vein thrombosis with lower extremity ulcers.
Upon her admission, she needed moderate assistance with walking and sitting/standing balance and total assistance with bed mobility and standing. She also needed substantial aid when bathing and dressing.
Connie received all three therapy disciplines––physical, occupational, and speech––five days a week to heal her wounds, increase her strength and balance, and improve her cognition and swallowing.
Despite three hospitalizations during her therapy program, Connie remained focused and still met her goals. Her wound improved, along with the associated pain, and she regained her independence with walking, sitting/standing balance, and bed mobility.
