After hospitalization for a fall that resulted in a neck fracture, William came to Life Care Center of Elyria for comprehensive inpatient rehabilitation.
Upon his arrival, William had significant weakness, pain, and low activity tolerance. He required assistance from two people for all functional mobility, total assistance with self-care tasks, and a lift to make transfers. He was also at risk for aspiration related to dysphagia, or difficulty swallowing.
The onsite physical, occupational, and speech therapists evaluated William, addressing his physical deficits, providing precaution education, and instructing him in adaptive techniques for completing daily tasks, pain management, and monitoring his swallowing (due to his risk of aspiration).
After working with therapists and addressing his needs, William managed to meet all his recovery goals and return home. At the time of his discharge, he was independent with self-care tasks and transfers and able to walk over 150 feet, including up and down steps. His pain was gone, and he was no longer at risk for aspiration.
"The therapists were in tune with what I needed, and my overall stay was good. There is a huge difference from where I started to where I am now! I achieved all my goals! I would come back here if I ever needed therapy again!" –– William

Ross is a bit different from our typical short-term therapy graduates, as he is just 26 years old. His experience shows that we are here to serve anyone who has lost their independence due to illness or injury––and that persistence pays off!
Here is what Ross had to say about his experience at Life Care Center of Westlake:
"Through Life Care of Westlake, I am managing my diabetes as well as checking my blood sugar levels and administering my own insulin. I am embracing my education and taking responsibility for my own health thanks to all of the help I received at Life Care. I have done so well here that now I no longer have to manage my diabetes with shots and my sugar levels are under control and able to be managed by medication. I am able to return home and engage in my favorite hobbies: the royal families and my writing.

Denise transferred to Life Care Center of Saint Louis for inpatient therapy after a stay at a long-term acute care facility, where she received treatment for a below-the-knee amputation.
At the time of her admission to Life Care, Denise required moderate assistance with bathing, lower-body dressing, and transfers. For wheelchair mobility, she required minimal assistance.
Denise’s rehabilitation program involved occupational therapy that focused on retraining her to perform activities of daily living and physical therapy that focused on strengthening her lower extremities and improving her transferring and balance.
During her therapy program, she transitioned to long-term care and received a prothesis to begin gait training.
After making great progress in her program, Denise discharged to a new home with her daughter. She is now independent with her activities of daily living, transfers, and wheelchair mobility. And she can walk short distances with minimal assistance while wearing her prosthesis.
“They really took good care of me. I was able to regain my independence and I can take care of myself now.” –– Denise

Rosemary arrived at The Oaks after a fall that resulted in fractures in her spine, ribs, and elbow.
Prior to her admission, Rosemary had been living with her husband in a split-level home, independent with self-care and mobility. After her fall, she could no longer bear weight on her upper extremities or perform functional tasks. She was also on an easy-to-chew diet with thick liquids.
The physical, occupational, and speech therapy teams at The Oaks partnered with Rosemary and developed a comprehensive plan of care. Physical and occupational therapies focused on improving her independence with mobility, self-care, and functional tasks via therapeutic exercise and gait training. Speech therapy focused on upgrading her to a regular diet.
With her determination and the help of the entire interdisciplinary team, Rosemary restored her mobility and independence with functional tasks, graduating to a supervised level. She upgraded to a regular diet and achieved a successful home safety evaluation. She is now back home with her husband!
