Deanna at Life Care Center of Elyria

Deanna came to Life Care Center of Elyria after receiving surgery for a bad fall. 

At admission, she had pain, decreased balance, limited range of motion, and reduced stranding strength and required total assistance with self-care and mobility. She could walk only five feet using a walker, with assistance.

To address her decreased ability, Elyria’s physical and occupational therapists developed for Deanna a thorough, patient-centered plan that focused on alleviating her pain, increasing her range of motion, strengthening her lower extremities, and improving her balance and gait to help her restore her mobility and functional independence.

After completing her therapy program, Deanna could walk 150 feet independently and complete self-care tasks without assistance. She returned home to The Abbewood Senior Living community, which is next door to Life Care Center of Elyria, with plans to continue outpatient therapy at Elyria to reinforce her long-term independence.

Alberto at Life Care Center of Yuma

Alberto was admitted to Life Care Center of Yuma with a sepsis diagnosis and had pain, decreased strength, and low tolerance for activity.

He required moderate assistance with all self-care tasks and was unable to walk on his own. He also needed help with all transfers.

Alberto participated in occupational therapy and physical therapy five times a week. Occupational therapy helped Alberto restore his upper-body strength and his ability to perform all self-care tasks. Physical therapy targeted Alberto’s lower-body strength, bed mobility, and walking ability.

After meeting all his recovery goals, Alberto was able to complete self-care tasks independently, transfer independently, and walk over 450 feet with crutches. The rehab team was overjoyed and proud to see him walking.

Alberrto plans to move to an apartment alone and is looking forward to receiving home health services to maintain his strength and functional independence.

Sarbjit at Life Care Center of North Glendale

Sarbjit arrived at Life Care Center of North Glendale with multiple ailments that caused him pain and reduced his ability to care for himself.

At admission, Sarbjit needed a moderate amount of aid with self-care tasks. He was able to get in and out of bed and transfer with contact assistance, and he needed minimal help walking with a cane.

He also struggled with a swallowing disorder and required thick liquids and an easy-to-chew diet.

Sarbjit received physical, occupational, and speech therapy services during his inpatient rehabilitation stay.

In physical and occupational therapies, Sarbjit focused on rebuilding his strength and reducing his fall risk with balance retraining and fall prevention education. In speech therapy, he focused on strengthening his swallowing ability and relearning to tolerate a regular diet. 

After just two weeks of therapy, Sarbjit discharged home to his family. He was independent with all self-care tasks and able to walk using a single-point cane without help. His swallowing was normal and allowed for a regular diet.
